Peer-reviewed case report documents 90% volume reduction together with extensive necrosis — achieved without surgery or full-dose chemotherapy after gemcitabine failed

SAN DIEGO, Sept. 7, 2026 /PRNewswire-PRWeb/ -- A peer-reviewed case report from ICCA CancerCare, accepted for publication, describes a marked radiographic response in a 56-year-old man with bulky, locally advanced pancreatic adenocarcinoma. The response was achieved without surgery or full-dose systemic chemotherapy. During treatment, the tumor liquefied and drained through the patient's existing biliary catheter.

The patient's disease had progressed over 17 months following three cycles of gemcitabine at a U.S. academic medical center. At intake on January 26, 2026, the tumor measured 101.3 × 93.7 mm. By May 13, 2026, it measured 87 × 72.9 mm — a 33% reduction in cross-sectional area and a 90% reduction in volume — with extensive internal necrosis. A follow-up PET/CT several months after injection showed only faint residual activity in the pancreatic head with no evidence of metastasis.

Treatment was delivered entirely on an outpatient basis at COFEPRIS-licensed facilities in Tijuana, Baja California, and comprised fractionated radiotherapy with radiosensitizing-dose capecitabine; image-guided intratumoral injection of an EpCAM/CD3 bispecific T-cell-engaging antibody; infusions of allogeneic natural killer cells and autologous lymphocytes; cord blood–derived mesenchymal stromal cells; and sequential pembrolizumab. To the authors' knowledge, this is the first published report of intratumoral EpCAM/CD3 T-cell engagement producing clinically evident tumor lysis in pancreatic cancer.
